By participating in this golf tournament, all players acknowledge and agree to the following terms:

1. Assumption of Risk: I understand and acknowledge that participating in golf tournaments involves inherent risks, including but not limited to, personal injury, property damage, and exposure to contagious diseases such as COVID-19.
2. Health and Safety: I acknowledge that the organizers have implemented certain health and safety measures to reduce the risk of exposure to contagious diseases, but I understand that these measures cannot eliminate all risks.
3. Personal Responsibility: I agree to take full responsibility for my own health and safety during the tournament. This includes adhering to any health and safety guidelines provided by the organizers, such as maintaining physical distance, wearing masks when required, and practicing proper hygiene.
4. Release of Liability: I hereby release, waive, discharge, and covenant not to sue the organizers, sponsors, volunteers, and any other entities associated with the golf tournament from any and all liability for any illness, injury, or damage arising from my participation in the tournament, including but not limited to exposure to contagious diseases.
5. Compliance with Rules and Regulations: I agree to abide by all rules, regulations, and instructions provided by the organizers throughout the duration of the tournament. Failure to comply may result in disqualification from the tournament and removal from the premises.
6. Acknowledgment of Understanding: I acknowledge that I have read and understand this At-Risk Disclaimer in its entirety, and I voluntarily agree to its terms and conditions. I understand that by participating in the golf tournament, I am assuming certain risks and releasing the organizers from liability.

Photo and Video Release Form for Golf Tournament Participants:

By checking the box below, I and team participants and spectators, hereby grant permission to Clubs and Holes and its representatives to take photographs and videos of me during my participation in the “Where the Holes At?” Golf Tournament held on May 18th at Buena Vista Golf Course. I understand that these photographs and videos may be used for promotional, marketing, and educational purposes, including but not limited to print materials, social media, websites, and other digital platforms.

I waive any right to inspect or approve the finished products, including written copy or electronic matter, that may be used in conjunction with these photographs and videos. I also waive any right to royalties or other compensation arising from or related to the use of the photographs and videos.

I understand that the photographs and videos may be used in perpetuity, and I release [Organizer’s Name], its representatives, employees, and affiliates from any and all claims, demands, or causes of action that I may have now or in the future arising out of or related to the use of the photographs and videos.
